Free In-Home Assessment
Here at Absolute Health Care, we offer Free in-home assessments for every potential client. Meeting face-to-face will allow us to work together to assess what the best plan of action will be for you and/or your loved ones.
Our Free In-Home Assessment will include the following:- One visit to the potential client's home scheduled for their convenience.
- A general assessment of the client, from their mental to their physical capabilities.
- A thorough health and safety assessment of every room in the client's home.

Personal Care Plan (PCP)
Having a Personal Care Plan (PCP) can take the weight off your shoulders. A PCP serves as a guide for a caregiver, so they know the patient's needs, and the level of care required.
A Personal Care Plan includes:
A unique plan tailored to the patient's personal health care needs. PCP's are always developed and supervised by a licensed nurse. PCP's include -- but are not limited to -- personal hygiene, meal prep, light housekeeping, exercise, and more. View service details -
Bathing, Grooming and Dressing
Bed Baths
Bathing is one of the most pleasant parts of the day, but for the elderly, it can also feel dehumanizing.
If you care for someone that needs help with bathing and hygiene, staying relaxed will help them feel at ease.
While you may think giving a bed bath requires medical assistance, any family carer can do this. The patient may feel more comfortable with you than a nurse.
Nurses and aides are available to bathe the patient or train you how to do it. Medicare might help to cover costs. Just ask the patient's physician.
Dressing
Give the patient enough time to do as much as s/he is able to on their own. This will build trust.
Let the patient decide what they want to wear. If they are easily confused, feel free to lay out different choices.
Shoes and slippers must always be well-fitting. They shouldn't have gum soles or laces as both can lead to accidents.
Grooming and hygiene
Make sure all grooming supplies (brush, comb) are within reach
Try to encourage the use of electric items when possible (toothbrush, shaver)
Instruct them to sit at or rest on the counter or sink to make doing things easier

Nutrition Guidance, Meal Preparation and Hydration Level Management
We're conscious of the fact that our patients have special dietary needs.
Caregivers can prepare meals following the guidelines set by an individual's physician and every meal is thoughtfully planned out with the patient's personal tastes in mind.
Patients are strongly encouraged to help out in the kitchen if they feel that they can. It'll help them to feel productive.
Ensuring a patient is hydrated is also deeply important. Some diseases (congestive heart failure) can limit the fluids the patient can absorb throughout the day so it's good to keep an eye on it.

Range of Motion and Strengthening Exercises as Allowed by Primary Care Physician
If a patient receives physical or occupational therapy, then AHC is able to assist patients with muscular exercises in accordance with the instructions provided by the patient's physician.
Primary Care Physician
A primary care physician (PCP) is a medical professional that practices general medicine. PCP's are the doctors or nurse practitioners that patients visit before being referred to medical specialists.
Choosing the right primary care physician can be a tedious process, however it's essential to choose a PCP that understands a patient's health condition in order to determine an appropriate health management plan. The right primary care physician will provide regular check ups and listen when you need to discuss concerns.

Medication Reminders and Vital Signs Monitored
Vital signs monitored throughout the day are:
- Blood Pressure
- Respirations
- Temperature
- Pulse/heart rate
- There are many different types of vital sign monitors (blood pressure and heart rate machines included).
There are also monitors that anesthesiologists use in the administering of anesthesia.
Another piece of equipment you might see by a patient's bedside is a monitor that's used to check the levels of oxygen in the patient's blood.
Medication Reminders
A massive part of caring for a patient at home is ensuring you are on top of your patient's medications. You need to know what time they need to take them and how many of each.
You might find it easy to make a note of everything by physically writing it down. That way you can use it as an organized checklist and can avoid the potential hazards of forgetting a dose.

Light Housekeeping, Laundry and Linen Changes
Cleanliness is essential to maintaining a happy mood and home.
We provide light housekeeping and laundry services to our patients.
These services include:
- Dusting
- Vacuuming
- Mopping
- Washing, Drying and Folding Laundry
- Linen changes throughout the house
- Having a dirty home can affect a patient's mood. As well as this, there are many health-related issues that can come from dust, mold or grime, particularly for the elderly.
As most elders prefer to be in their own home as they age, we like to ensure that we provide these services at their disposal.
Understandably, as a caregiver, your time is already running on empty. That's why we suggest getting help around the home so you can spend more of your time with your loved one.

Transportation
We understand that not every patient has means of transport available, which is why we offer assistance with it when needed.
Our transport services can provide trips to the store, appointments, or social endeavors such as church, family gatherings, etc.
While there are limits, we will go over these on a one-to-one basis with the patient and/or their caregiver.
Services with access for patients restricted to a wheelchair is also provided.
The satisfaction of the patient is what we care most about, and so our drivers are expertly selected to ensure they bring a jovial and positive experience to the patient.

Companionship and Security Assurances
Socializing is an important aspect of you/r patient's life. It can make them feel good and keep them feel like they're still part of the world.
As people, we're social creatures by nature, even those of us who aren't particularly outgoing. It's nice for a person to have the choice as to whether or not they see other people.
Nobody should be left to feel lonely.
Our caregivers make sure to provide companionship for patients who might seek more socialization.
These companionship services can include things like:
- Taking walks together
- Conversations about life or events
- Playing card/board games together
- Assistance with hobbies
- It's important that the caregivers are not only attentive but enjoy the company of their patient. We like to know that the patient doesn't feel alone and that there's no danger of an accident.
Other activities or hobbies people enjoy can continue to happen with the aid of a companion. The patient can continue to enjoy the things they like without feeling as though they don't have the opportunity to.

Respite Care for Family or Care Provider
Rule number one of being a carer is you should never feel guilty for needing a break.
Being a caregiver, by default, is one of the most selfless things anyone can be. That said, as a caregiver, you should get time to yourself. You deserve it and your loved one does too.
Our respite programs are put in place for these reasons. We have caregivers that can come in during your respite to fill in for you.
Respite Care
We understand that humans are complex and guilt is an issue, but a short break is necessary for the carer's peace of mind.
However much we love the person we're caring for, regular breaks from any profession are a necessary part of life.
Daily care can take a toll on you: from your socialization to your mental health. Your taking a break can also be beneficial for your loved one in that you'll return with a fresh mind.
